MISI Community Solutions is a community-based healthcare organization dedicated to helping individuals and families achieve healthier, more stable lives. Through Housing Navigation, Enhanced Care Management (ECM), Community Health Worker (CHW), Doula, Recuperative Care, and other health-related services, we connect eligible Medi-Cal members with the resources and support they need to overcome barriers and improve their quality of life. Working in partnership with healthcare providers, hospitals, health plans, case managers, and community organizations, we deliver compassionate, person-centered care that addresses both health and social needs.
