Jake's Journey Home's mission is simple: Connect. Respect. Assist.
We connect with unhoused persons—veterans or community members—through respect and coordination of services. Providing wraparound services, including inpatient care, day treatment, mental health support, therapy, medical assistance, job training, vocational skills, higher education opportunities, and meeting basic needs such as food, DMV assistance, social security support, and access to essential cards. Additionally, we offer peer support to facilitate a lifepath change. These services ensure that each person's tomorrow is better than today. On January 15, 2026, Jake’s Journey Home achieved a major milestone with the establishment of our Community Assist Center—a sustainable solution for individuals at risk. The center provides essential wraparound services, including mental health support, inpatient care, job training, education opportunities, and assistance with basic needs.
Guided by our commitment to Connect, Respect, and Assist, the Community Assist Center serves as a welcoming space where individuals can access critical resources, rebuild stability, and take meaningful steps toward self-sufficiency and a brighter future.
